I not sure what "have to" refers to but you find already upgraded weapons so if you want to use them maybe they ment that or the game actually forces your to upgrade at some point. But I also read that you should keep it as low as possible and only upgrade if you have to. Is the equip rank generated on you highest piece? Of so then yes you should actually have all at the same rank.

One person said that on some tips for the game on nightmare, I read it here or somewhere else. My guess is not but it sounded like upgrading actually works against you so the enemies become stronger but your gear is not that much better when upgraded. Not like Dark Souls where you actually can study and practice for bosses.
